Telehealth Consent

By completing the NuvaMed intake form, you confirm that you have read, understood, and agreed to this Telehealth Informed Consent. This consent is required before any telehealth services may be provided.

1. What Is Telehealth?

Telehealth refers to the delivery of healthcare services using electronic communications and information technology, including asynchronous online consultations (also known as "store-and-forward" telehealth). Through NuvaMed, you will complete a health intake questionnaire that will be reviewed asynchronously by an independent licensed physician in your state.

Telehealth is not the same as an in-person visit. The physician will not physically examine you. Their evaluation will be based on the health information you provide in your intake form.

2. Nature of the Telehealth Consultation

The telehealth consultation provided through NuvaMed's platform is:

  • Asynchronous — you complete a questionnaire and a physician reviews it on their schedule, typically within 24–48 hours
  • Limited in scope — the physician evaluates whether the requested treatment is appropriate based on your submitted health history
  • Not a replacement for a comprehensive in-person medical examination or your primary care provider
  • Conducted by an independent licensed physician, not by NuvaMed itself

3. Potential Benefits

Telehealth services may offer the following benefits:

  • Convenient access to licensed medical care from your home
  • Faster access to treatment for appropriate conditions
  • Reduced cost compared to traditional in-person visits
  • Privacy and discretion for sensitive health matters

4. Potential Risks and Limitations

You understand and acknowledge that telehealth services carry certain risks and limitations, including:

  • The physician cannot physically examine you, which may limit their ability to identify certain conditions
  • Delays in evaluation due to technical issues, physician availability, or information gaps
  • The possibility that the physician may determine that telehealth is not appropriate for your situation and that an in-person evaluation is needed
  • The possibility that a prescription may not be issued if the physician determines it is not clinically appropriate
  • Risks associated with compounded medications, which are not FDA-approved
  • Electronic transmission of health information carries inherent privacy risks despite security measures

5. Your Responsibilities

By consenting to telehealth services, you agree to:

  • Provide complete, honest, and accurate health information in your intake form
  • Disclose all current medications, supplements, allergies, and relevant medical history
  • Seek emergency medical care (call 911 or go to an emergency room) for any life-threatening emergency — do not use telehealth for emergencies
  • Follow up with your primary care physician or a specialist as recommended
  • Read all product information and follow your physician's instructions for any prescribed medication
  • Report any adverse reactions or side effects promptly by contacting us at help@nuvamed.org

6. Not for Emergencies

NuvaMed's telehealth services are NOT appropriate for medical emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Do not use this platform for chest pain, difficulty breathing, signs of stroke, severe allergic reactions, or any other life-threatening condition.

7. Privacy of Health Information

Your health information is protected and handled in accordance with our Privacy Policy and applicable federal and state law, including HIPAA. Your information will only be shared with the treating physician and, upon prescription, with the compounding pharmacy for fulfillment purposes.

8. Voluntary Participation and Right to Withdraw

Your participation in telehealth services is entirely voluntary. You have the right to withdraw your consent at any time by contacting us at help@nuvamed.org. Withdrawal of consent will not affect any care you have already received. Please refer to our Refund Policy for information about refunds upon withdrawal.

9. State-Specific Requirements

Telehealth laws and regulations vary by state. The independent physicians on our platform are licensed in your state and will comply with all applicable state-specific requirements for telehealth practice. In some states, additional consent or in-person visit requirements may apply.

10. Acknowledgment

By proceeding with your intake, you acknowledge that:

  • You have read and understood this Telehealth Informed Consent
  • You understand the nature, benefits, and risks of telehealth services
  • You agree to the terms described above
  • You are at least 18 years of age
  • You have had the opportunity to ask questions and have had them answered to your satisfaction
